About Lifescore Foundation

Lifescore Foundation is a Black, Indigenous, and People of Color (BIPOC)-led organization committed to empowering individuals and families on the south side of Chicago to thrive through education, support, and community-building.

Our History and Background

Lifescore Foundation was founded in February 2019 by Beyonca Johnson, our current President. We prioritize serving marginalized communities and creating opportunities for those who have been historically underrepresented and underserved.

 

Through our programs and services, we seek to empower and uplift individuals and communities, while promoting equity and social justice.

Our Programs and Services

Lifescore Foundation offers a wide range of programs and services that support the growth and development of underserved communities. Our programs are designed to empower individuals to reach their full potential and to help build stronger, more resilient communities. Here are some examples of our programs and services:

Financial Literacy

We offer workshops and one-on-one counseling to help individuals improve their financial management skills and achieve their financial goals.

Youth Empowerment

Our youth programs provide mentorship, leadership training, and college readiness support to at-risk youth in the community.

Working at Home

Workforce Development

We offer job readiness training, resume writing assistance, and job placement support to help individuals build their skills and find employment.

Happy Senior Couple

Senior Citizens Support

Our senior citizens program provides transportation assistance, healthcare advocacy, and social activities to promote healthy aging and independent living.

Trauma Support

We offer trauma support and violence prevention programming to help individuals heal from past trauma and build resilience for the future.

Partnerships and Collaborations

At Lifescore Foundation, we believe in the power of collaboration. We partner with other organizations and individuals to achieve our shared goals and to better serve the community. Some of our partners include local schools, community centers, and healthcare providers. We also collaborate with other nonprofits and community groups on joint projects and initiatives

Secretary

Rocky is a successful entrepreneur and business leader with over 20 years of experience in community development. He is committed to promoting equity and social justice and oversees our partnerships and collaborations with other organizations.

President

Beyonca is the founder and President of Lifescore Foundation. She has over 10 years of experience in nonprofit management and is passionate about creating opportunities for BIPOC communities to thrive.

Dr. Monica Stout

Board Member

Dr. Stout is a healthcare professional and licensed therapist with experience in community-based mental health services. She is passionate about creating safe and inclusive spaces for marginalized communities and provides guidance on our programs and services.

Keisha is a Healthcare Director with a passion for improving the health and well-being of underserved communities. She provides expertise on our senior citizens program.
